• The Mentoring Parent Consent Form: the parent will need to be able to sign this electronically and submit it online.

• The CHAMP Student Profile: the student will need to be able to fill it out electronically and submit it online.

• CHAMP Pledge: the student will need to be able to fill it out electronically and submit it online.

Once you have filled out the 3 documents and saved your responses, please submit them to this link (make sure you are logged into your Google Share Account in Launchpad):
